Death Identification Service Nationwide Solution

PERS is seeking a solution that allows the agency to transmit pension roll as batch file fact-of-death queries to participating vital records offices and receive fact-of-death reports in response. Also have an ability to access solution functions that allow a search and or query by individual.

PERS pays retirement benefits to which need to end upon the death of a benefit recipient. Death records are needed to identify and verify deaths to prevent overpayment and retirement benefits.

Vendor shall provide an independent third-party death registration source that draws from additional death registration sources to which PERS currently does not have access. PERS must be able to do individual lookups, as well as run all of PERS' benefit recipients against the database of reported deaths to identify any current benefit recipients not previously identified as deceased.

PERS population

• 420,000 members
• 168,000 benefit recipients

Estimated frequency of batch transmittal of up to once per month. Estimated number of individual searches a month = 400. Access to these records will be approximately ten (10) PERS staff members.

The successful Offeror must comply with applicable PERS information security policies.

Training Requirements

Awardee must provide training and support for PERS staff for their solution provided.

Transition Services

At PERS's option, the awarded vendor will perform any requested transition services which may include analysis, data migration, configuration, implementation services, consulting services, and providing data to PERS in a suitable format via electronic submission. The awarded vendor shall provide PERS with full access and possession of all created materials, data, and records that pertain to the services described in this solicitation.

**Submission method:** All submissions must be emailed only to the Authorized Representative listed above. Responses must be received on or before **Tuesday, November 24, 2026, 2:00PM PST**.

**Evaluation:** Those meeting the requirements will be evaluated to determine the "Best Value" for the agency. A maximum score of 100 points may be obtained:

• Implementation and Schedule of the proposed solution = 15 points
• Offeror's organization, work experience, and relevant successful projects = 15 points
• Cost = 30 points
• Solution Demonstration = 10 points
• Meets solution requirements = 30 points

Agency will apply an OSBE preference by increasing the score or point total of any Offer submitted by an OSBE by 10%.

**Insurance:** Prior to execution of the Contract, the apparent successful Offeror shall secure and demonstrate to Agency proof of insurance coverage meeting the requirements identified in this solicitation or as otherwise negotiated. Successful Offeror shall obtain at successful Offeror's expense the required types of insurance specified in Attachment # 7.

**Term of Service:** PERS anticipates the award of one Contract from this solicitation. The initial term of the Contract is anticipated to be for a four (4) year term with optional annual renewals up to (10) ten years.
