RFP Field Damage Repairs

The City of Fort Worth seeks proposals to finalize an Agreement for Field Damage Repairs for the Water Department on an "as needed" basis.

**Submission method:** All submissions/Proposals must be submitted electronically prior to the close date and time under the respective Project/RFP via the Euna Portal: https://fortworthtexas.bonfirehub.com/portal/?tab=openOpportunities

Failure to complete and submit all required information listed in the respective RFP will be grounds for rejection of a Proposal as non-responsive. No late Proposals shall be accepted. Proposals delivered in any other manner than using the Euna Portal will not be accepted or considered.

All questions, explanations, clarifications, or interpretations desired by Proposers regarding any part of the solicitation must be electronically submitted to the City of Fort Worth's Euna Portal through the "Vendor Discussion" section under the respective Project/Request For Proposal (RFP) before the Questions Due Date and time specified in the Euna Portal.

Insurance required:

• Commercial General Liability: $1,000,000 each occurrence; $2,000,000 aggregate.
• Automobile Liability: $1,000,000 each occurrence on a combined single limit basis.
• Worker's Compensation: statutory limits; Employers' liability $100,000 bodily injury by accident, $100,000 bodily injury by disease per employee, $500,000 bodily injury by disease policy limit.
• Professional Liability (Errors & Omissions): $1,000,000 each claim; $1,000,000 aggregate.
