The City of Hazlehurst ("City") is seeking sealed bids for the Southside Baptist Culvert Replacement project (Southside Baptist Church Lot, 35 Alma Hwy, Hazlehurst, GA 31539).

3. Remove & replace 1 EA of 8" concrete headwall (10 ft x 4 ft)
4. Backfill 1.94 CY of washed-out area with Unclassified Fill Dirt (9.5 ft x 5.5 ft x 1 ft)
5. Install 4 EA of 22 ft thermoplastic stripes, 4.5" wide (88 LF)
Invoices must include at least 4 Photos of "Before" and 4 Photos "After" work completion. Debris must be hauled to a landfill. Landfill tickets are required to be submitted with an invoice.
The contractor will restore any landscaping, concrete, etc. disturbed during construction to its original state or approved alternative. Any damage to the City's property caused during construction will be repaired at the contractor's expense.
Bids and all required documents must be submitted online to the DFS Procurement Portal at dfs.bonfirehub.com no later than Tuesday, November 10, 2026 at 2:00 P.M., local time (EST). Bids received after the due date and time will be considered non-responsive and will not be considered.
Submission of any bid signifies the Contractor's agreement that their bid and the content thereof, are valid for ninety (90) calendar days following the submission deadline.
