ITB 26-006 Hydrovac & CCTV Project

The City of Doraville is seeking a qualified and experienced Contractor for hydro-vac cleaning of stormwater pipes and adjacent structures (if any), CCTV inspection of the pipes shown in Table 1 of Exhibit B in the attached documents, and potential CIPP and CCCP of City-selected bid alternate pipes.

The Contract will be to provide services to the City in accordance with the Contract Documents. All work shall be performed in accordance with the Scope of Services attached hereto as Exhibit B.

Service Provider agrees to furnish all skill and labor of every description necessary to carry out perform, perform the services in accordance with the Contract Documents (the “Work”).

It shall be the sole responsibility of the Service Provider to provide for all tools, parts, and equipment necessary to perform work under this Contract.

Any work required beyond that which is specified herein shall be reported in advance to the City. At no time shall work beyond the scope be performed without prior written authorization from the City.

**Submission method:** Bids shall be submitted in hard copy format (two originals) and one flash drive. Any bid submitted in any other format (email, fax, mail, etc.) will not be accepted. Bids shall only be accepted at the City Hall at: 2421 Van Fleet Cir, Doraville, GA 30360. Bids shall be accepted prior to the due date, but not later than the due date and time.

All questions or requests for clarification must be sent via e-mail only to gabidding26@keckwood.com. Questions and clarifications will be answered in the form of an addendum. Any addenda, schedule changes, and other important information regarding the solicitation related to this solicitation will be posted on City website at www.Doravillega.us.

The City of Doraville reserves the right to reject all bids and to waive technicalities and informalities, and to make an award in the best interest of the City of Doraville.

The contract period for this contract is **60 days** from the date stated in the Notice to Proceed Letter.

Insurance

• Commercial general liability insurance with coverage of not less than ONE MILLION DOLLARS (**$1,000,000.00**) combined single limit per occurrence.
• Automobile liability insurance with policy limits of not less than ONE MILLION DOLLARS (**$1,000,000.00**) combined single limit per accident or occurrence covering each motor vehicle operated on City property.
• The policy limits of the Service Provider’s employer’s liability insurance shall not be less than $100,000 for “each employee.”
• Professional Liability insurance in the amount of at least ONE MILLION DOLLARS (**$1,000,000.00**) per claim.
