The work is covered by the Information for Bidders, Bid Agreement, General Conditions and Technical Specifications, and includes the following divisions:

• Division 02 - Existing Conditions: selective demolition and removals
• Division 05 - Metals: cold-formed metal framing
• Division 06 - Wood, Plastics, and Composites: miscellaneous rough carpentry and finish carpentry
• Division 07 - Thermal and Moisture Protection
• Division 08 - Openings: hollow metal doors and frames, door hardware, safety and security films
• Division 09 - Finishes: gypsum board assemblies, tiling, acoustical ceilings, resilient flooring, painting
• Division 10 - Specialties: signage, plastic toilet compartments, toilet accessories
• Division 12 - Furnishings: countertops
• Division 22 - Plumbing
• Division 23 - Heating, Ventilating, and Air-Conditioning (HVAC)
• Division 26 - Electrical
• Division 27 - Communications: nurse call systems
• Division 28 - Electronic Safety and Security: zoned fire-alarm system

Sealed bids will be received by the New York City Health and Hospitals Corporation, on the day, at the time, and in the room specified in the Advertisement for Bids appearing in the City Record, where they will be publicly opened and read aloud.

The *Bid Breakdown Form* is for determination of responsibility only. All spaces must be completed. If a column does not apply, enter a zero or N/A. Failure to fill in these spaces may result in the bid being declared non-responsive.

The solicitation documents include a *Bid Submission Package* with a bid page, form of bid bond, bid breakdown, prior experience reference forms, an MWBE utilization plan, and payment and performance bonds.

This work is covered by a Project Labor Agreement (2021 NYC Health + Hospitals Renovation Project Labor Agreement, 2021 - 2025). All Contractors performing Program Work are required to sign a *Letter of Assent*.
