The Department of Human Services (“DHS”) is seeking a contractor to provide Miscellaneous Ambulance Services, to include: Advanced Life Support (“ALS”), Basic Life Support (“BLS”), and non-emergent ambulance services for the Office of Mental Health & Substance Abuse Services (“OMHSAS”), Clarks Summit State Hospital (“CSSH”), located at 1451 Hillside Drive, Clarks Summit, Pennsylvania 18411.

**Submission method:** Responses are submitted electronically through the PA Supplier Portal at www.pasupplierportal.state.pa.us.

• Log on with the User ID and Password created when registering as a supplier, and choose the Bidder tab.
• Enter the Solicitation Number in the appropriate field and select the Apply button, then click the blue Event Number link to open the Solicitation.
• Download all required attachments. Bid documents are found under the Header tab in the Notes and Attachments section.
• Select Register to register your company for any potential change notices, then select the Create Response button.
• Under the Header tab, select Questions to answer two **mandatory** questions. Select “Yes” from the Reply drop-down menu on each one to indicate that you have read and understood the listed attachments.
• Enter line item prices in the Items tab in the Price column per unit of measure, unless a price sheet was completed as an attachment.
• Before submitting, select the Summary tab to review, click the Check button, then click the Submit button.

A submitted response can be edited an unlimited number of times before the bid deadline by opening the Response Number link and clicking the Edit button. A submitted bid can be withdrawn by clicking the Withdraw button.
