Wall-mounted, double-roll toilet tissue dispenser.

• Capacity: Two (2) rolls of standard-core toilet tissue.
• Maximum roll diameter: 4-1/2 inches.
• Dispenser shall feature a sliding door or equivalent mechanism that provides access to the next roll when the first roll is depleted.
• Material: Durable plastic suitable for institutional/commercial restroom use.
• Color: Smoke gray or equivalent.
• Approximate dimensions: Width: 10-3/4 inches; Height: 6-1/2 inches; Depth: 4-7/8 inches.
• Wall-mounted design.
• Finish: Polished or equivalent.
• All mounting hardware necessary for installation shall be included.
• Dispenser shall accommodate standard-core toilet tissue.
• Dispenser shall be suitable for frequent institutional use and routine cleaning.

Vendor Requirements:

• Vendor must provide a specification sheet with the bid.
• The specification sheet must clearly demonstrate compliance with the requirements listed above.
• Bids submitted without the required specification sheet may be deemed non-responsive.

Quotes must be electronically submitted to **PHLContracts**. In no event will the City accept a Quote from seller unless City has received a duly completed and signed *Consent and Authorization Agreement* by the Submission Date specified in PHLContracts.

The contract will be awarded to the lowest, responsible, responsive Seller. All terms and prices quoted shall be valid for 60 days from the Submission Date. All products must be delivered, at the prices quoted, FOB Destination Point.

The term of the Contract shall commence on the Effective Date in PHLContracts (Effective Date) and shall terminate automatically 365 days thereafter.

General Liability Insurance: Limit of Liability: **$500,000** per occurrence combined single limit for bodily injury (including death) and property damage liability. Workers' Compensation – Statutory limits.
